If you are eligible and would like to book a Kambo ceremony, please follow these instructions:

• Are you currently fit & healthy? (If not, please specify)
• Do you have any health conditions?
• Are you on any medications?
• Do you have any serious physical / mental struggles in day to day life?
• Have you had any psychotic episodes or mental breakdowns in the past?
  
• Do you have any severe alcohol/drug addictions?
• Have you had the Covid vaccine in the last 4 weeks?
• Do you want a 1-to-1, a private group ceremony or join onto a large group ceremony?

Please answer all questions as honestly as possible. (For my safety as well as yours)
